You gain +3 hit points. For every Hit Die you possess beyond 3, you gain an additional +1 hit point. If you have more than 3 Hit Dice, you gain +1 hit points whenever you gain a Hit Die (such as when you gain a level).

Basically, this feat provides 1 additional Hit Point per level and applies both proactively and retroactively, just like changes to Constitution score behave. Thus it makes little difference whether the perk is taken early on or later.

However the 1 HP increase per level is not that great of a benefit to justify a perk point, especially if the the class does not get that many bonus feats to begin with. Dealing damage and dispatching enemies quicker will always be more important for the long-term survival than tanking damage, and even then 1 additional HP per level is not going to improve survivability noticeably. It also worth noting that spells like Bear's Endurance or Aid can contribute to a more noticeable increase in HP in combat without costing a precious feat point.

Nevertheless, Toughness has one important use and that is being a prerequisite for the Stalwart Defender prestige class - those who plan to branch out should plan adopting the feat accordingly.
